Morpheus8 vs microneedling: which one you actually need
Many people who ask about Morpheus8 are better suited to microneedling with PRP. The choice usually comes down to your main concern. Loose skin (jowls, neck, crepey cheeks) or deep acne scars: Morpheus8. Texture, tone, pores, fine lines, or early scarring: microneedling with PRP, with less downtime and lower cost. The key difference is that Morpheus8 adds radiofrequency heat under the skin, and microneedling doesn’t.
What Morpheus8 and microneedling each do
Morpheus8 at our Arlington Heights office is the InMode Morpheus8, a fractional radiofrequency microneedling handpiece. Insulated needles release bipolar radiofrequency from their tips, so the heat lands in the dermis and the upper layer of fat while the surface is mostly spared. The heat contracts tissue on the day and builds collagen over months, per Hendricks and Farhang in the Journal of Cosmetic Dermatology. The FDA cleared it for procedures “where coagulation/contraction of soft tissue or hemostasis is needed,” per the 510(k) summary.
Microneedling with PRP is a motorized pen that makes thousands of controlled micro-injuries in the dermis and leaves the surface mostly intact, which triggers collagen, per Alster and Graham in Dermatologic Surgery. The PRP is your own blood, spun in the room and applied during and after. The FDA has cleared microneedling pens for facial acne scars, facial wrinkles, and abdominal scars in adults 22 and older, per its microneedling devices page. No PRP product is FDA-approved. It’s made from your own blood, not a drug.
The two work on the same principle, and heat is the main difference. Heat tightens, but it also costs more, is more uncomfortable, keeps you red longer, and carries risks the pen doesn’t, so we recommend it only when you need it.
Which to choose, by concern
Jowls, a soft jawline, neck crepe, or cheeks starting to sag? Morpheus8 is the one to consider. Microneedling doesn’t tighten loose skin, however many sessions you do.
Acne scars? Both can help. A meta-analysis of 14 controlled studies (472 patients) found microneedling plus PRP roughly tripled the odds of a greater than 50 percent improvement compared with microneedling alone, per Kang and Lu in Frontiers in Medicine. A randomized split-face trial found radiofrequency microneedling matched fractional CO2 laser for atrophic acne scars after three sessions, with a shorter recovery and fewer pigment problems, per the trial. For rolling and boxcar scars on skin that’s otherwise firm, we start with microneedling with PRP. For deep scars, or scars on skin that’s also loose, we’d suggest Morpheus8. Microneedling won’t remove deep ice-pick scars on its own.
Texture, pores, fine lines, or dullness? Microneedling with PRP is usually the better choice. Morpheus8 does these too, but you’d be paying for heat you don’t need. If this is your main concern and your skin is still firm, you likely don’t need Morpheus8.
Melasma or darker skin? We generally recommend microneedling. As an add-on to topical treatment it improved melasma severity at 12 weeks in a 12-study meta-analysis, per Bailey and colleagues in JAAD, and radiofrequency doesn’t target pigment. Morpheus8 can be used on all skin types with the energy turned down, but its clearance limits energies above 62 mJ per pin to skin types I to IV, per the 2023 510(k).
There’s a third option for mild laxity with no needles and no downtime: Forma, radiofrequency delivered from the surface, which we treat as the step before Morpheus8.
What to expect for downtime
Morpheus8. On day 0, you’ll be red, warm, and swollen, with a visible grid of needle marks. Over days 1 to 3, the redness and swelling settle, and tiny scabs form at the needle sites. From days 3 to 7, the scabs slough off and your skin feels dry and tight. RealSelf members report one to three days of social downtime, per RealSelf, patient-reported.
Microneedling with PRP. On day 0, you’ll be red and warm, like a sunburn, and the pinpoint bleeding stops before you leave the room. Expect redness for about 24 to 48 hours and light flaking by days 3 to 5, and it’s best to skip makeup for the first 24 hours, per RealSelf, patient-reported.
For planning, microneedling on a Friday usually means pinkness on Saturday that has faded by Monday. After Morpheus8 on a Friday, tiny scabs are often still visible on Monday, so schedule it when a few days of visible recovery won’t be a problem.
Sessions, time in the room, and cost
A Morpheus8 appointment takes 60 to 90 minutes for a full face, numbing included, and longer with the neck. The numbing cream sits for about an hour. Published series use three sessions, 4 to 8 weeks apart, and milder laxity may need fewer. We assess results three months after the last session.
Microneedling with PRP starts with a blood draw, followed by 20 to 30 minutes of numbing while the PRP spins, then 20 to 30 minutes of needling, about 60 to 75 minutes in all. Acne-scar trials use 3 to 6 sessions about 4 weeks apart.
Morpheus8 costs more per session, and each session does more. Microneedling with PRP costs less per session and usually takes more of them. Neither is permanent, so plan for upkeep: yearly maintenance is common for Morpheus8, and every 6 to 12 months for microneedling. Individual results vary.

Which one hurts more, and which one carries more risk
Morpheus8 does hurt more, which is why numbing cream comes first. Each stamp is pressure and heat, described as “typically mild to moderate and well-tolerated” in a 2026 systematic review of 20 studies, per Kumar and colleagues. Microneedling was ranked the most tolerable acne-scar treatment for pain in a network meta-analysis of 68 randomized trials, per Wu and colleagues in PeerJ.
Risk is the bigger difference. Both can cause redness, swelling, a cold sore flare, or dark marks in darker skin. Morpheus8 adds the risks that come with heat. On October 15, 2025 the FDA issued a safety communication on radiofrequency microneedling citing reports of burns, scarring, fat loss, disfigurement, and nerve damage. Those are voluntary reports, not rates. We use conservative depth and energy in thin-fat areas like the temples and under the eyes, because fat loss is a reported complication. Microneedling with PRP has no heat to misplace, and the meta-analyses found no serious adverse events.
