These terms cover the Skinovatio Arlington Heights website (skinovatioarlingtonheights.com), our contact form, online booking, and the text messaging program run by 255 MEDICAL PLLC, doing business as Skinovatio Arlington Heights, 3375 N Arlington Heights Rd, Suite C, Arlington Heights, IL 60004. By using the site, booking, or agreeing to texts, you accept these terms.
1. Our text messaging program
Skinovatio Arlington Heights sends text messages about your inquiries and appointments, including:
- Replies to questions you send through our contact form
- Appointment confirmations, reminders, changes, and cancellations
- Requests for information we need before your visit
- Replies when you text us with a question
- Promotional texts about offers, only if you separately opted in to them when booking
2. How you opt in
Contact form. Our contact form asks for your mobile number and, directly under it, has an optional checkbox that is unchecked when the page loads. If you check it, you agree to receive text messages from Skinovatio Arlington Heights at that number about your inquiry and appointments. If you leave it blank, we won’t text you about your inquiry and will call or email you instead.
Online booking. Our online booking portal is run by Pagoda Labs, Inc. (“Pagoda”) for us. The booking step asks you to agree to appointment texts: confirmations, reminders, changes or cancellations, and requests for information needed before your visit. These are informational and are not used for marketing. Promotional texts are a separate, optional choice, and agreeing to them is never a condition of booking or of any purchase. You can always book by calling (847) 637-5882 instead.
Your consent is given directly to Skinovatio Arlington Heights. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
3. Message frequency and cost
Message frequency varies with your inquiries and appointments. Message and data rates may apply, depending on your mobile carrier and plan. We are not responsible for carrier charges.
4. Opting out and getting help
Reply STOP to any text to opt out. You may get one final text confirming it, and then we won’t text you again unless you opt back in. Reply HELP to any text for help, or call us at (847) 637-5882.
5. Carriers
Carriers are not liable for delayed or undelivered messages. Texts work with most major U.S. carriers but are not guaranteed on every carrier, device, or plan.
6. Health information by text
Please don’t send medical details by text. We’ll go over those privately at your consult. Our texts are not medical advice. In an emergency, call 911.
7. Privacy
Our Privacy policy explains what we collect, how we collect text message consent, and how we use and store it. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.
8. Appointments
Online booking asks for a card to hold your appointment. It isn’t charged when you book. We only charge a $50 fee if you miss your appointment, arrive more than 15 minutes late, or cancel within 24 hours. Prices on this site are “starting at” prices and can change. Your provider confirms your treatment and price at your consult.
9. Using this website
Information on this site is general and is not medical advice. Individual results vary, and a consultation with a licensed provider comes first. Please don’t misuse the site or the contact form, for example by sending spam or booking in someone else’s name without permission.
10. Changes and contact
