What the appointment is like
Plan on about 45 minutes, and a good part of that is waiting on the centrifuge. You’ll relax with numbing cream under your eyes while the blood spins. Then the PRF goes in through a cannula or a fine needle, and it feels like pressure rather than a sting. Your nurse practitioner works quickly, since PRF starts to gel within minutes.

We’ll examine your under-eyes in natural light, do a pinch test to tell skin from fat, and recommend whether PRF, filler, or a surgical opinion fits best. You’ll leave with a written plan.
Blood draw and spin
A few tubes from your arm, just like a lab visit. They spin at low speed for several minutes while numbing cream sits under your eyes.
Injection
The fibrin is drawn up and placed under each eye through a cannula or fine needle. Most people feel pressure, not pain.

Dermal filler under the eyes is immediate and predictable. Three products are approved by the FDA for infraorbital hollowing: Juvederm Volbella XC, Restylane Eyelight, and RHA Redensity Eye. Volbella XC “can last up to 1 year,” per Juvederm, and Galderma reports Eyelight benefits through 18 months, per the Restylane Eyelight page. Filler is the better choice for a deep hollow with good skin over it. In crepey skin, though, the gel can sit visibly under the surface, and in the wrong candidate it can look puffy for a year.
Microneedling with PRP treats the surface of the whole face: texture, pores, and fine lines. It doesn’t reach a hollow. If your main concern is overall texture, microneedling is a good place to start. If it’s tired-looking under-eyes, start with PRF. Many people do both in the same visit with the same tubes of blood.

Any injection under the eye carries a small risk of injuring a blood vessel, and unlike HA filler, PRF can’t be dissolved. Across 14 studies in a 2025 review, no serious adverse events were reported; common effects are swelling, bruising, and tenderness.

What it treats
Under-eye concerns usually fall into three groups, and PRF helps some more than others.
Crepey lower-lid skin: fine horizontal lines and a crinkled texture when you smile. This is where PRF has its best data. A 2025 systematic review of 14 studies found “PRF was associated with improvements in skin texture, wrinkles, and crepiness,” per Sollitto and colleagues in the Journal of Cosmetic Dermatology.
Dark circles. If the darkness is brown pigment in the skin itself, PRP has the stronger evidence, per the same review, so we’d recommend PRP. If it’s a bluish shadow through thin skin, PRF’s thickening effect helps gradually over a series.
Mild hollowing: a shallow tear trough that shows as a shadow at the inner corner. PRF gives a gradual, modest improvement. A deep trough needs volume, and for volume we’d recommend dermal filler, specifically one of the three products approved for the under-eye.
“Baggy eyes” can describe two different problems. Crepey, tired, slightly hollow under-eyes are about skin quality, and that’s what PRF is made for. A bag that’s there all day at the same size is usually orbital fat that has pushed forward. PRF doesn’t remove fat. A systematic review of under-eye treatments put it plainly: “Blepharoplasty surgery is best when excessive skin laxity is the underlying cause,” per Michelle and colleagues in Dermatologic Surgery. That calls for a surgical consult, and we’ll recommend one at your first visit if that’s what we see.
The same blood draw can treat more than the under-eyes. Injected PRF or PRP in the cheeks, smile lines, and neck is the same menu item, aimed at skin quality. It doesn’t add volume or tighten. If texture is a concern across your whole face, microneedling with PRP covers more ground with the same tubes of blood. The same PRF can also go into the scalp as an add-on to PRP hair restoration.
How it works
PRF is platelet-rich fibrin. We draw a few tubes of blood from your arm and spin them at low speed without an anticoagulant, and the result sets into a soft fibrin gel packed with platelets and white cells. The gel releases growth factors (PDGF, VEGF, TGF-beta) slowly over days rather than all at once, per Davies and Miron in Periodontology 2000.
Under the thin skin of the lower lid, those growth factors stimulate fibroblasts and new small vessels over weeks. The skin thickens and smooths a little. The gel itself also gives a short-lived volume effect that fades over days to weeks, so the fullness you see the day after isn’t your final result. Your real result shows up around week 4 to 8.
PRP is the older, liquid version: spun faster, with an anticoagulant. PRP has the stronger evidence for pigment-type dark circles, PRF has the better data for texture, and the head-to-head studies are small. A 2025 review of 96 articles found “Only few studies have compared PRP versus PRF with all demonstrating superior outcomes using PRF,” and its senior author founded a PRF company, so that finding should be read with caution. We’ll recommend the option that fits your under-eye concern.
PRF is not FDA-approved for any cosmetic use. It’s your own blood, prepared in a centrifuge kit that is FDA-cleared only for making platelet concentrate, per the FDA’s clearance for one such kit. Using it under the eyes is off-label. You’re welcome to ask us which kit we use.
Afterwards
Downtime and aftercare
The main downtime to plan for is bruising, which can last up to a week. The under-eye shows bruising more than anywhere else on the face. Skip concealer for the first 24 hours.
Day 0: expect puffiness under both eyes from the volume of fluid under thin skin. Use a cold compress that evening, and sleep with your head propped up the first night.
Days 1 to 3: the swelling settles. If you’re going to bruise, it will show now. Avoid makeup over the needle sites for 24 hours, avoid rubbing, and skip alcohol and ibuprofen for a day if your nurse practitioner agrees.
Days 3 to 7: any bruises turn yellow and clear, and concealer is fine by now. Keep this window in mind if you have photos or events coming up.
Week 2 onward: there’s nothing to avoid. There’s no sun restriction after PRF either, which sets it apart from most treatments on our menu.

Often, yes. The under-eye skin is thin and shows a bruise more than anywhere else on the face. Bruising tends to appear on days 1 to 3, turn yellow, and clear by about day 7, and you’ll be puffy on day one from the volume of fluid. We suggest booking at least a week before any event or photos, and pausing aspirin, ibuprofen, and fish oil beforehand if your doctor agrees.
Not true bags. If the bag is a pocket of orbital fat pushing forward, or loose lid skin folding over, PRF won’t remove it, and a surgical consult for blepharoplasty is the better path. If what you’re calling a bag is crepey, tired, slightly hollow skin that reads as puffiness, PRF can help over a series of three sessions.

PRF is made from your own blood, and your body metabolises it. If you don’t love the result, it resolves on its own over months. Filler under the eye would need dissolving, while PRF simply fades away.
