What to expect
What the appointment is like
Plan on thirty to forty-five minutes. Topical numbing goes on while we prepare the vial, and the mix itself has lidocaine in it. We place the product with a needle or cannula in the deep layers of the skin in a fan or grid across the cheek and temple. You’ll feel pressure and a dull ache, and you’ll look fuller walking out than you will in a week. That’s the water in the mix.

We judge the first session at four to six weeks and the series at three months after the last one. In the cheek trial, 66 percent of people were responders at month 7 and 77 percent were still responders at month 24. The label states results lasted up to 25 months for some people.

If one cheek is flat and the rest of the face is fine, a syringe of filler is faster, cheaper and reversible. If the face has thinned everywhere, Sculptra is a better fit than treating each hollow separately with filler. If the problem is descent, tissue that has slid rather than shrunk, PDO threads can move it, while Sculptra doesn’t. And if the skin itself is loose and crepey over the volume loss, Morpheus8 changes the skin while Sculptra restores what’s under it. Those two are often staged a few months apart rather than chosen between.

Sculptra can’t be dissolved. There’s no enzyme the way HA has hyaluronidase. Papules and nodules were each reported in about 8.6 percent of people in one study, most felt rather than seen; careful technique and the 5-5-5 massage rule lower the risk.

What it treats
Sculptra treats a face that has deflated all over, rather than a single spot. That difference helps determine whether Sculptra or filler is the better choice. You can compare options on our dermal fillers page.
Sculptra Aesthetic is approved by the FDA for correction of fine lines and wrinkles in the cheek region (2023), and for nasolabial folds and other facial wrinkles where a deep cross-hatch injection technique is appropriate (2009), per the FDA summary of safety and effectiveness. In practice, that means a midface that has flattened as facial fat pads shrink with age.
Injectors commonly use it for hollow temples and the area in front of the jaw. Those sites go beyond the folds and cheek in the label, so we describe them as facial hollows and discuss this at your consult.
This was Sculptra’s original 2004 approval, and it’s still an indication. If that’s your situation, please let us know at your consult, because the plan and the vial count differ.
We never place it in the lips. The label says “Do not inject into the red area (vermillion) of the lip.” We also don’t use it around the eyes, where the label says it has not been studied, or on the buttocks, hips, chest or arms. A body indication exists in Europe as of December 2025, but it’s Europe only. Body Sculptra in the US is off-label, and we don’t offer it.
How it works
The label explains it this way: “SCULPTRA Aesthetic works by initially filling a wrinkle with small PLLA beads. As the beads biodegrade the body may produce new collagen where SCULPTRA Aesthetic is injected,” per the Galderma instructions for use.
The microparticles act as a scaffold. They trigger a controlled response in the skin, fibroblasts build collagen around them over weeks to months, and the particles themselves break down and clear. What’s left is your own tissue, not a gel. That’s why there’s nothing to see for the first month, and why there’s no enzyme to dissolve it if you change your mind.
The product comes as a powder that’s mixed with sterile water, often with lidocaine, before it’s injected. The 2021 label update allows higher dilution and immediate use, and the label study found the lidocaine version “was well tolerated, caused less pain” than the older mix, per Galderma.

Downtime and aftercare
Day 0: expect swelling, redness, tenderness, and a face that looks fuller than it will be. Days 1 to 7: swelling and bruising settle, and the fullness drops away as the water is absorbed. Weeks 2 to 6: nothing visible yet while collagen forms. This is expected. In the cheek study, tenderness (93.5 percent), bruising (93.5 percent), swelling (87.1 percent) and pain (83.9 percent) were the most common events, and the majority resolved within 14 days, per the FDA summary linked above.
The most important aftercare step is massage. As the label puts it: “It is recommended that the patient should massage the treated areas for five minutes, five times per day for five days after the injection session.” That’s five minutes, five times a day, for five days. It spreads the particles evenly, and it’s the main defense against the small bumps described below. A phone reminder helps.
Beyond that, use ice for swelling, hold off on makeup until the needle sites close, and avoid hard exercise, alcohol, saunas or facials for a day or two. Wear sunscreen, and take the usual bruise precautions beforehand: it’s best to skip aspirin, ibuprofen, fish oil and alcohol for a few days before if your doctor agrees, and please never stop a prescribed blood thinner.

Up to about two years for some people. The label says results “for some subjects lasted for up to 25 months after the last treatment session,” and in the cheek trial 76.9 percent of people were still responders at month 24. It’s not permanent, and a single maintenance session every one to two years is common. Individual results vary.
Most people need two to four vials across two or three sessions, one or two vials per session, at least three weeks apart. A younger face with early temple hollowing needs fewer. A face that has deflated across the cheeks, temples and folds needs more. Charles will estimate your count at the consult, and the price card shows the per-vial cost.
Filler is a gel that adds volume the same day, treats one spot precisely, and can be dissolved. Sculptra is a powder that triggers your own collagen over months, treats diffuse deflation, and can’t be dissolved. If you want a change the same day, filler is the better choice. If you prefer a gradual, all-over restoration, Sculptra is the better fit.
Visible results take about a month. Day-one fullness is water and disappears within days. The first real change shows at four to six weeks after a session, and the series is judged three months after the last one. Galderma reports 96 percent of trial patients showing improvement at three months by investigator assessment.

No. Poly-L-lactic acid has no reversing enzyme the way hyaluronic acid has hyaluronidase. What’s built is your own collagen. That’s why we’re conservative with vial counts, stay out of the lips and eyes, and use hyaluronic acid in areas where reversibility matters.
Some, yes. Topical numbing goes on first and the mix contains lidocaine, so most people describe pressure and a dull ache rather than sharp pain. The face feels tender and full for a few days, and the massage can feel uncomfortable the first day or two, which is normal. Bruising is common and fades within a week or two.
Sculptra isn’t reversible. It works by prompting your own collagen over months, and it can’t be dissolved. That’s why we go one vial at a time with months in between, rather than aiming for the full result in a single visit.
