Treat a double chin, no surgery

Kybella is a prescription injection of deoxycholic acid, approved by the FDA to improve moderate to severe fullness from fat under the chin in adults. Injected in a grid across the fat pad, it destroys fat cells, which the body clears over weeks. It’s given in two to six sessions at least a month apart.

The number depends on how many vials each session takes, which is set by the size of the fat pad, and on how many sessions you need, usually two to four. A small pad takes fewer vials and fewer rounds, and a fuller one takes more of both. Every treatment starts with a consultation and a written plan, and we’ll estimate your vials there.

The fat cells Kybella destroys don’t grow back, so the reduction is long-lasting. The cells that remain can still enlarge if you gain weight, which can refill the area. The label’s word is “improvement,” and trials followed patients for up to six months after the last treatment. If you keep your weight steady, the result should hold.

Most people plan on two to four sessions, four to six weeks apart, and some need the full six the label allows. In the trials, 59 percent of subjects had all six. The number depends on the size of the fat pad and how much you’d like gone. Sessions can’t be closer than a month apart.

For the approved use, under the chin, in adults with fat rather than loose skin, yes, with common local side effects: swelling, bruising, pain, and numbness in most people. The serious but rare risk is temporary nerve injury along the jaw, which happened in 4 percent of trial subjects and resolved on its own in a median of 44 days. It isn’t approved or recommended anywhere else on the body.

It does, for about ten minutes. The injections burn and sting while the drug works, and then the area feels full and tender. Ice and numbing beforehand help take the edge off. RealSelf’s patient reports call it a slight burning sensation lasting about ten minutes, with a range from painless to severe. Soreness afterward is mild and lasts a few days.

If your concern is a soft, pinchable pocket of fat under the chin and your skin still has snap, the trial data say most people see a meaningful change: 70 percent improved at least one grade after up to six sessions, versus 18.6 percent on placebo. If your concern is loose skin or a heavy neck, it isn’t, and skin tightening or a surgical consult is the better option.

Liposuction removes more fat in one procedure and a neck lift can also fix skin and muscle bands, at the cost of surgery, anesthesia, and real recovery. Kybella is a series of short office visits with no incisions and no sedation, for a smaller, softer pocket with good skin. If you have a large pad or loose skin, a surgeon is the way to go. If you have a small pad, decent skin, and would rather avoid surgery, Kybella may be a good fit.

It depends on where the beard sits. The label lists hair loss at the injection sites as a warning and says to hold further treatments until it resolves, so a full beard under the chin is something we’ll discuss before we inject. We’ll look at where your beard sits at your consult and plan around it.

You can expect one to two weeks. It’s largest in the first three days, settles over days 4 to 14, and some firmness and numbness linger for weeks. In the trials, swelling lasted past 30 days for 20 percent of people and numbness for 42 percent. It helps to schedule sessions when you don’t have major events in the following two weeks.

Kybella isn’t reversible. Deoxycholic acid destroys the fat cells it reaches, and they don’t come back. That’s how the treatment works, and it’s why we start conservatively and reassess a month later rather than using four vials in the first session.
