PRF under the eyes: how long it lasts, and what the first month looks like

Results from PRF under the eyes typically last about six months. That’s shorter than many sources suggest, but it’s what the research supports: in the largest review of PRF and PRP around the eyes, 14 studies in all, the improvements in texture and crepiness had often faded by month six. So we plan PRF under the eyes as a series of three sessions about four weeks apart. We assess results at three months and schedule maintenance when they fade.

Why we plan on six months, not a year

PRF is platelet-rich fibrin, your own blood spun slowly and without an anticoagulant, so it sets into a soft gel that releases growth factors over days instead of hours, per Davies and Miron in Periodontology 2000. Nothing foreign goes in. That’s a big part of its appeal, and it’s also why it doesn’t last like a filler. A filler is a gel that sits there until your body breaks it down. PRF stimulates your own skin to make collagen, and that effect fades over time.

The most useful summary of how long results last is a 2025 systematic review of 14 studies on PRP and PRF for the under-eye area, per Sollitto and colleagues in the Journal of Cosmetic Dermatology. It found “PRF was associated with improvements in skin texture, wrinkles, and crepiness, while PRP showed stronger evidence for treating hyperpigmentation,” and that “PRF improvements often diminished by 6 months.” No serious adverse events were reported across those studies.

Six months is the number we plan around for PRF under the eyes at our Arlington Heights office. Some people’s results hold longer. Individual results vary.

Your first month after PRF, day by day

Day 0. The appointment is about 45 minutes: a blood draw, a spin in the centrifuge, then the PRF goes in under each eye through a cannula or a fine needle, with topical numbing. It feels like pressure. PRF has to be injected soon after spinning because it starts to gel. You’ll leave puffy under the eyes, because a volume of fluid is sitting under thin skin. Parking is in the free lot right outside.

Days 1 to 3. The swelling settles. Bruising, if you’re going to get it, shows up now. A cold compress helps, and it’s best to sleep with your head up the first night. Avoid makeup over the needle sites for 24 hours, avoid rubbing, and skip alcohol and ibuprofen for 24 hours if the nurse agrees.

Days 3 to 7. Any bruises yellow and clear. This is the main downtime, which is why we suggest booking at least a week before an event.

Weeks 2 to 3. The early fullness fades, which is expected. The gel gives a short-lived volume effect that goes away over days to weeks, and any “filling” you saw on day one was never the result.

Weeks 4 to 8. Skin quality starts to change as new collagen forms: less crepe, a smoother texture, a less tired look. Session two lands at about week 4 and session three at about week 8.

Month 3. This is when we assess the series result. That’s when the studies assess it, and it’s when we take your after photo.

Why not filler, if it lasts longer?

They solve different problems, and the wrong choice under the eyes can affect your result for months.

Three fillers are approved by the FDA specifically for under-eye hollows: Juvederm Volbella XC, Restylane Eyelight, and RHA Redensity Eye. Allergan says Volbella XC can last up to a year, per Juvederm’s site, and Galderma reports Eyelight benefits through 18 months, per the Restylane Eyelight product page. Filler gives immediate, predictable volume, and a hyaluronic acid filler can be dissolved with hyaluronidase if it goes wrong.

PRF gives gradual, subtle change in the skin itself, with no filler lump and no bluish Tyndall tint under thin skin. It can’t be dissolved, because it’s your own tissue, and like any under-eye injection it carries a small risk of vascular injury.

So the decision comes down to what’s going on under your eyes. A deep hollow with good skin over it calls for dermal filler. Crepey, dull, tired-looking skin with mild hollowing is a good match for PRF. PRF also suits people who’d rather not have a product placed under the eyes. If you want an immediate change, filler is the better choice.

What PRF won’t fix, and what can help instead

Baggy eyes are one of the most common under-eye concerns. If your bags are pockets of fat pushing forward under the lid, or loose lid skin folding over, PRF won’t fix them. A systematic review of under-eye treatments put it plainly: “Blepharoplasty surgery is best when excessive skin laxity is the underlying cause,” and fillers or fat grafting work best for dark circles from volume loss, per Michelle and colleagues in Dermatologic Surgery. In that case, we’ll recommend seeing a surgeon.

If the texture problem runs across the whole lower face rather than just under the eyes, microneedling with PRP can treat it in the same visit, using the same blood draw.

Timing PRF under the eyes in Arlington Heights

There’s no sun restriction after PRF, so it can be done any time of year. The main thing to plan around is bruising, which can last up to a week after each session, so schedule sessions at least a week before holiday events or other important dates.

Does PRF under the eyes last longer than PRP?

Not quite. If anything, it’s the reverse. In the 2025 systematic review of 14 studies, PRF’s texture improvements “often diminished by 6 months,” while PRP’s pigment results “were sustained at similar intervals.” PRF has the better data for crepiness and fine lines, PRP for dark circles from pigment. The few small head-to-head studies leaned toward PRF, but the samples were small and one review’s senior author founded a PRF company, so choose based on what’s going on under your eyes rather than which lasts longer.


Do I need more than one PRF session?

Yes, we recommend a series. The studies mostly used three sessions about four weeks apart, and single sessions give little lasting change. Collagen builds slowly, and each session adds to the last. We assess the series at three months, then plan maintenance around month six.


Can PRF under the eyes make bags worse?

It’s normal to look puffier for a few days, because a volume of fluid is sitting under thin skin. That settles by about day 3 and isn’t the result. PRF can’t create or remove fat bags. If your bags are true fat pads or loose lid skin, PRF won’t fix them, and we’ll recommend a surgeon’s opinion instead.


When can I wear concealer after PRF?

You can wear it over the needle sites after 24 hours. Bruising can last through day 7, so you may want concealer through the week. Apply it gently without rubbing, and skip it if the skin over a site is still open.
